Provide assessments, care planning, and service coordination, besides providing direct care to each patient and instruction/supervision to HHAs, PCAs, and other caregivers; Responsible for medication prep and administering Intramuscular and subcutaneous injections, ostomy care, urinary catheterization, wound care, negative pressure therapy, pain management, and other treatment options; Responsible for INR monitoring, PICC line dressing changes, disease management; Ensure quality and safe delivery of home health services; Provide quality and timely documentation after providing care; Maintain and update clinical knowledge on an ongoing basis, and conduct research to improve patient outcomes and healthcare processes.

POSITION REQUIREMENTS:  The position requires a Bachelor of Science degree in Nursing or related or Foreign Equivalent. License/Certification: Registered Nurse (RN) license to practice in New York state is required. Must have one year of experience in the job offered or related position. Must have one (1) year of experience delivering medications via oral, IV, and intramuscular injections, monitoring patient’s post-operative vitals, developing a strategy to target nursing and patient satisfaction, improving response and patient care quality, and promoting quality and safety initiatives.

RATE OF PAY: $82,867 per year

HOURS: Mon-Fri, 8AM-5PM, 40 hours per week.
